Articulation Agreement (sample)
An example of just one way that a high school and a local college or university can agree to give graduates credit as they continue their education. Partners are encouraged to discuss and develop their own unique agreement that best suit the needs of their students.
Bridge FAQ Becoming FRMCA
Employer Agreement
This documents the roles and responsibilities of the student, the employer, the school, and the student's parent or guardian, and contains space for all of these parties' signatures. In order to satisfy the work experience requirements of the ProStart program, a signed copy of this agreement must be on file at the school, and student must adhere to the agreement throughout their ProStart work experience(s).
Exam Administration Handbook
The procedures for requesting, distributing and returning exams are outlined in this handbook that is updated each school year.
Exam Return Information Sheet
The information that needs to accompany your completed exams. This envelope is provided when you order your exams.

Kitchen Lab Evaluation Rubric
Use this helpful Kitchen Lab Scoring Rubric to make evaluating student performance quick and consistent.
Mentor Survey
This form asks mentors and worksite employers for their feedback on their experiences in the ProStart program. This is a tool intended to be used to fine tune and improve the program.
ProStart Press Release (sample)
Use this to promote the ProStart program in your community, encouraging students, parents, and foodservice operators to become involved.
Reference Request (sample)
Students can use this to request reference letters from former employers.
Request for Final Exam
Use this form to request the ProStart final exam master from the NRAEF.
Research Request Form
Use this form when you want to request that your exams be re-scored or if there are any problems with your exam results.
Student Letter of Recommendation (sample)
A rough sketch of a letter from a student's mentor or worksite employer to future employers. It should be fleshed out with information specific to each student.
Student Work Experience Checklist
Included in the activity book, this checklist can be utilized when working with industry and students.
Student Workplace Validation Form
Used by the state restaurant association ProStart program coordinator, this form evaluates students' work experiences according to the requirements outlined by the National Restaurant Association Educational Foundation (NRAEF). Students must satisfy these requirements in order to be eligible for the ProStart National Certificate of Acheivement. The completed form must be signed by the state restaurant association ProStart program coordinator and submitted to the NRAEF in order to release the students certicate.
